For a ration card form download, go to your state food and civil supplies portal, open its Forms or Downloads section and save the PDF for your task, such as a fresh card, member inclusion or a correction. Forms are made by each state, so a form from another state will not be accepted.
Many states have moved fully online, where the form is filled on screen and there is no PDF at all. Others still want a printed form with photos and signatures, handed in at the supply office. Form 4, Form 8 and Form 14: the numbered forms
Numbered forms people search for, like ration card form 4 or 14 no form, come mostly from West Bengal, where the food department gives each service a form number.
- Form 3: a new family that has no digital ration card yet.
- Form 4: adding a member to a family that already has a card.
- Form 5: correction in an existing card.
- Form 7: to give up the card altogether.
- Form 14: shifting one person out to a new family.
Form 8 is also searched often; read the heading printed on the form itself before using it, since numbers can be revised. Ration card self declaration form PDF download and filling
Many states ask for a self declaration with a new card or category request. In it you state that the family's income is within the limit, that nobody holds another card, and that the details are true. Some states print it as the last page of the main form; others keep it as a separate PDF in the downloads list.
Fill your name and the head of family's name the same way on both the form and the declaration. Where a witness or local representative signature is needed, get it before you go to the office so you do not have to come back.
Ration card online form in states without PDFs
In several states the paper form has been replaced by an online application, and the only download you get is the acknowledgement. Tamil Nadu takes new card and change requests on its smart card portal; Haryana builds cards from the Family ID record, so the form is really the Family ID update; Bihar routes requests through its citizen services portal. Ration card form status after you submit
A form status check needs the acknowledgement or receipt number. Online forms show it on screen and send it by SMS; offline forms carry it on the counter slip.
