A ration card download Assam option for ordinary card holders is limited: the state department for Food, Public Distribution & Consumer Affairs publishes reports and the Proforma C form online, but fresh cards, duplicates and changes are still handled in person by the FPD&CA staff who serve your area. Ration card form C Assam PDF download
Proforma C is the department's form for a new card and for most changes. It is linked from the Apply for Ration Card page on the department site. Fill the printout in block letters and pin the papers your case needs behind it. The same page lists which papers go with each request: a new card, a separate card for a split family, adding or deleting a member, a change of head, a change of shop or address, and a duplicate card.
Digital ration card details through Mera Ration
Mera Ration belongs to the Union food ministry. Verify with card number or Aadhaar and it lists your card, its members and the month's grain. For Assam families it is the nearest thing to a soft copy. The app can also show nearby shops. If the app cannot find the card, the Aadhaar may not be seeded, which the dealer or FPD&CA office can check.
Adding a member or transferring a card in Assam
Babies, brides, deaths, a different shop or a move across districts: each is recorded on Proforma C and lodged with FPD&CA. The department lists these papers:
- Inclusion or deletion: original card, an identity proof, proof of date of birth, and a surrender certificate for inclusion or a death certificate for deletion.
- New child: birth certificate and a plain paper request.
- Marriage: a deletion certificate from the FPD&CA authority of the earlier address.
- Change of shop or address: original card and photo identity.
We found no working citizen web route for adding members or ration card transfer online in Assam, so budget a trip to the office.
eKYC rule for NFSA cards in Assam
The department has made Aadhaar eKYC compulsory for all NFSA members. The dealer records it by thumb scan. Skipping it puts a member at risk of deletion, and cards idle for months were pulled up for review in the recent drive. If a member's fingerprint fails, ask the dealer about other ways to verify or visit the FPD&CA office.
