MP ration card download works differently from most states: Madhya Pradesh gives eligible families a patrata parchi, or eligibility slip, linked to the Samagra family ID, and that slip is what you print from the Ration Mitra portal of the food department. What is the MP patrata parchi
In many states the ration card itself is the main paper. In Madhya Pradesh, after a family is found eligible through the Samagra database, the food department issues a patrata parchi. This slip, sometimes searched as ration card parchi download, is proof that the family can draw ration under NFSA. It carries the Samagra family ID, the members, the category such as Antyodaya or a priority household group, and the fair price shop. Treat it as your e ration card in MP. MP ration card download, step by step
Menu wording on Ration Mitra changes and is often in Hindi, so look for words such as पात्रता पर्ची, eligibility slip or beneficiary. If you only see reports, pick your district first and then look for a search box for the Samagra ID. On a phone, rotate the screen if tables are cut off. Save the slip right away; do not depend on reopening the page later.
Samagra ID: the base of every MP card
Every MP ration benefit starts with the Samagra family ID and the individual member IDs. If a member is not in Samagra, they cannot appear on the slip. Use the Samagra portal to check family members and their eKYC. Births, marriages and deaths must be updated in Samagra first, which rural families do via the panchayat secretary and town families via the ward office, before the ration record can change.

There is no single form that creates a ration card in MP. The family first registers in Samagra, then applies for eligibility with proof of the eligible category your family falls in at the Gram Panchayat, ward office or a Lok Seva Kendra. Ration card e KYC online in MP
