A ration card online apply Assam route that takes the whole form on the web could not be confirmed by us: the department site lets you download Proforma C, but the completed sheet, with supporting papers, is lodged with the FPD&CA authority for your district or sub division (civil). New NFSA cards in the state have also been given out in special drives.

Ration card online apply Assam: what is really online
Searches often promise a full online form for Assam. On the department site, the online part is the form download and the information pages. The application itself is handed in at the office. Some private sites show screens from an older sign up page or name other portals; we could not confirm any of them as a working public route. If the department starts an online form, it will show on the Apply for Ration Card page first, so check there before visiting.
People also ask about a last date. Assam has issued new cards in rounds, and dates are set per round by the department, so check with the Gaon Panchayat or FPD&CA counter whether a round is running.
Who can apply for an Assam NFSA card
The department page says the eldest adult woman of a family who is an Indian citizen can apply for a new NFSA card, if the family meets the beneficiary rules and income limits the department sets. If the family has no adult woman, the eldest adult man may apply with a written certificate from the village head, Gaon Panchayat president, ward commissioner, FPD&CA inspector or similar authority, usually issued after an affidavit.
AAY or PHH is fixed by officers after scrutiny; the form has no box to choose.

Separate card, duplicate card and other requests
Proforma C also covers other cases. A family splitting from the parents' card needs the parents' original card, a no objection certificate from its head, a self declaration and a village or ward certificate, along with birth, residence and photo proof. Replacing a lost card calls for a signed declaration and the police complaint copy. Changing the head of family needs the original card and, if the head has died, the death certificate. These all go to the same FPD&CA office.
After you submit: tracking and the first ration
That dated acknowledgement matters. Assam has no confirmed public tracking page for these forms, so follow up at the office with that receipt.
